Reporting an occurrence

When reporting an occurrence you can fill in a standardized form asking you a couple of questions:

  • Is the reporting of this occurrence mandatory
  • What is the type of occurrence report
  • When did the occurrence occur
  • Describe the occurrence
  • Damage report on aircraft and third party
  • Were there any injuries?
  • What was the worst possible consequence?

After the user reported the occurrence, the safety manager will get a notification.

Mitigation

Allowing other users to propose mitigations provides real learning opportunities for the team. The assigned user proposes a mitigation with a description, cost, time frame, and a final risk level index. The safety manager can then reject or accept the mitigation.

Each mitigation action is then becoming an action that needs to be implemented before the indicated due date. The task is added to the dashboard of the assigned user.

Frequently asked questions

Common questions about the Safety Management module.

Is the Aviatize SMS module ECCAIRS-compliant?

Yes. The Safety Management System follows the ECCAIRS protocol for standardized occurrence reporting, ensuring your reports meet European aviation safety information sharing requirements.

Can incident reports be submitted anonymously?

Yes. When reporting an occurrence, the submitter can choose to anonymize the report. The safety manager receives the full report without identifying information, encouraging honest reporting without fear of repercussions.

How does Aviatize handle safety mitigations?

After an occurrence is reported, team members can propose mitigations with descriptions, cost estimates, timeframes, and risk level assessments. The safety manager reviews and accepts or rejects proposals. Accepted mitigations become tracked action items with due dates on the assigned user dashboard.
